Employee Responsibility
Responsibility of Employee for FMLA:

(1) Determine if you are eligible.(FMLA Overview)

(2) Complete Request for FMLA form and return it to your supervisor.

(3) Submit Certification of Health Care Provider form to your health care provider. Upon completion, return it to your supervisor, preferably with the Request for FMLA form. Be sure to return it by the deadline given in the letter from your supervisor. If unable to do so, contact your supervisor prior to the deadline to discuss. Failure to return your Certification by the deadline could jeopardize your FMLA status.

(4) Verify your paid leave balances with your timekeeper.

(5) Confirm written receipt of FMLA coverage from your supervisor.

(6) Upon release from your health care provider to return to work, bring the Certification of Fitness for Duty form to your supervisor on date of return.
