Employee Responsibility

Responsibility of Employee for FMLA:

(1) Determine if you are eligible.(FMLA Overview)

(2) Complete Request for FMLA form and return it to your supervisor.

(3) Submit applicable Certification Form to the appropriate party. Upon completion, return it to your supervisor. Be sure to return it by the deadline given in the letter from your supervisor. If unable to do so, contact your supervisor prior to the deadline to discuss. Failure to return your Certification by the deadline could jeopardize your FMLA status. For a qualifying exigency caused by military duty, a copy of active duty orders is required.

(4) Verify your paid leave balances with your timekeeper.

(5) Confirm written receipt of FMLA coverage from your supervisor.

(6)If applicable, upon release from your health care provider to return to work, bring the Certification of Fitness for Duty form to your supervisor on date of return.
